This page serves as a guide to assist LMC community members in choosing the correct reporting form to share their concerns, in order to ensure efficient and effective response from the CARE Team. The CARE Team (Concern, Assessment, Response, Evaluation) is a multidisciplinary group that supports students of concern by coordinating early intervention and campus resources.
These forms are not intended for emergency response; if you are experiencing an emergency, imminent risk, or a threat of violence, first call 911, then contact Campus Safety at (269) 470-6084, and after you are safe, complete the appropriate report. Submitting a report does not replace contacting Campus Safety or the police.
Submitted referral forms are processed during normal business hours and are not monitored after hours, on weekends, or during college closures.
