Incident Reporting

If you are experiencing an emergency involving immediate risk of harm to yourself or others, first call 911, then contact Campus Security at 269-470-6084, and after you are safe, complete the appropriate report.

We appreciate your partnership in supporting campus wellness. By submitting a report, you are helping us maintain a healthy and safe campus community. Join us in taking care of our community by creating a culture of reporting.

This page serves as a guide to assist LMC community members in choosing the correct reporting form to share their concerns, in order to ensure efficient and effective response from the CARE Team. The CARE Team (Concern, Assessment, Response, Evaluation) is a multidisciplinary group that supports students of concern by coordinating early intervention and campus resources.

Immediately call 9-1-1- or 6911 if a student is unable to care for themselves or is severely impaired due to substance use. 

If there is suspected substance abuse or misuse or the student shares that they are struggling with substance abuse or misuse, please submit a Care and Concern Report.

Start by speaking with the student directly to better understand the situation. Ask whether they are having difficulty with the course material or if personal, financial, health, or other concerns are affecting their ability to learn.

Encourage the student to connect with Lake Michigan College academic support resources, such as advising, success coaching, or tutoring.

If concerns about the student’s academic progress persist, submit an academic alert in TargetX to initiate additional outreach and support.

Repeated or ongoing disruptions, like frequent interruptions or distracting behavior, should be addressed and reported. Submit a Student Code of Conduct Violation Report for continuous or hostile behavior. If the situation becomes violent or dangerous, call 9‑1‑1 and Campus Safety (269‑470‑6084) immediately.

Use minor disruptions as an opportunity to set clear expectations and build rapport, and submit a Care and Concern Report if personal or academic challenges appear to be contributing.

Encourage the student to seek help from the LMC Student Well-Being & Accessibility Department.

Follow up by submitting a Care and Concern Report so that the CARE Team can follow up with the student to offer support and resources.  

Encourage the student to think about some of their interests/hobbies and look at the various ways to get involved through LMC Student Clubs. The next time you see the student, check in with them again to see if their feelings have resolved.

If you feel outreach is appropriate, submit a Care and Concern Report so that our CARE Team can follow-up with the student to offer support and services.

 

Stress, anxiety, and feelings of depression are normal feelings that all of us experience at times. If a student shares with you that they are stressed, anxious, or depressed, be supportive and validating to the student. Perhaps ask them what is causing them to feel this way and talk with them about how they could cope with their stress. Encourage them to find some effective ways to address their feelings (organization, exercise, asking for help, or sharing with a trusted friend). 

If the student shares that they are experiencing stress, anxiety, or depression and indicate it is impairing normal daily functioning, encourage the student to seek help from the LMC Student Well-Being & Accessibility Department.  

If you feel follow-up is needed after talking with the student, please submit a Care and Concern Report so that our CARE Team can follow up with the student to offer support and resources.

If the student has communicated a specific plan to harm themselves or others, this is an EMERGENCY SITUATION – CALL 9-1-1 or 6911. 

Please follow up by completing a Care and Concern Report so that our CARE Team can follow up with the student after they receive the required immediate medical and safety care.  

Let the student know you are grateful they felt comfortable sharing this information with you. Also, let the student know you will need to share this information with the Title IX Coordinator, so that they can be offered additional support and services.  

Submit a report to the LMC Title IX Coordinator either online at Title IX Report or by contacting the Dean of Student Life & Engagement, Title IX Coordinator at (269) 927-6172.

First, contact the student directly. Send them an email to let them know you are concerned and would like to meet with them.  

If the student is not responsive to your outreach attempts, you can submit a notification via TargetX.

If you are worried about the student’s overall personal, physical, mental, or emotional wellbeing, please submit a Care and Concern Report so the CARE Team can follow up and connect the student to support and resources.   

Refer students to Student Well-Being & Accessibility, the designated campus authority, to verify disabilities and to authorize specific accommodations for students with documented disabilities.

These forms are not intended for emergency response; if you are experiencing an emergency, imminent risk, or a threat of violence, first call 911, then contact Campus Safety at (269) 470-6084, and after you are safe, complete the appropriate report. Submitting a report does not replace contacting Campus Safety or the police.

  • Academic Dishonesty (Instructor Use) - For instructors to report suspected academic dishonesty, such as plagiarism, exam fraud, unauthorized materials use, or other violations of academic integrity in a course.
  • Care & Concern Report - For anyone in the LMC community to report concerns about a student’s well-being, behavior, or basic needs so the CARE Team can provide supportive outreach and interventions. This form is for non-conduct concerns; student conduct violations should be reported through the Code of Conduct Violation Reporting Form.
  • Code of Conduct Violation (Student) - For reporting alleged violations of the Student Code of Conduct, such as disruptive behavior, substance use, or other prohibited actions. Reports are reviewed through the student conduct process and are separate from CARE referrals, which focus on supportive, non-conduct concerns.
  • Crime/Campus Security Reporting - For faculty, staff, students, or community members to report alleged crimes so Campus Security and the Clery Compliance Officer can review and take appropriate action. This form is for criminal incidents and is separate from CARE referrals and student conduct reports.
  • Guest/Visitor Incident Report (Non-Housing Related) - For reporting inappropriate, hostile, or disruptive behavior by non-student visitors or guests on campus. This form is used for non-emergency incidents involving guests/visitors.
  • Sexual Harassment/Title IX Issues - For any member of the LMC community to report incidents of sexual harassment, sexual misconduct, or discrimination under Title IX policies. This form may be used by individuals who have experienced or become aware of such concerns and supports both reporting and access to supportive measures.
  • Student Academic Complaint - For students to report concerns related to academic issues in a course, such as grading concerns, instructional practices, classroom environment, or academic integrity concerns that have not been resolved directly with the instructor. This process is reviewed by Academic Staff.

Submitted referral forms are processed during normal business hours and are not monitored after hours, on weekends, or during college closures.
